Residential Customer Owned Tank Agreement:

AGREEMENT DATE: Sept. 1, 2000 - April 30, 2001

Green Bay Gas hereinafter referred to as "Supplier" and (your full name), hereinafter referred to as "Consumer," do hereby enter into this "Pre-Purchase Agreement" hereinafter referred to as "Agreement", as follows:

Supplier will provide to the Consumer (number) gallons of propane for the guaranteed sum of 79.9 cents per gallon, to be delivered between Sept. 1, 2000 and April 30, 2001; said deliveries of propane to be made to Consumer by Supplier on an "automatic delivery" basis. The account will be placed on "will-call" delivery after the contract has ended unless notification is received from consumer.

Total Guaranteed Pre-Purchase Agreement amount:
$(number of gallons X (current price of propane(in cents) / 100)

Payment to Supplier by Consumer shall be as follows: One third of total Pre-Purchase Agreement amount due upon execution of Agreement, an additional one third of Agreement amount due within 30 days of execution and the final one third due on or before April 30, 2001. Any payment not received on or before its due date shall be considered to be in default of this Agreement and will cause Agreement to be retroactively terminated.

Any monetary amount tendered prior to default shall at Consumer option:
1. Remain on account as a credit balance.
2. Be refunded by Supplier within 90 days of default Iess an administrative surcharge of $125.00.

Upon exhaustion of Agreement gallons ((number)), Consumer will be invoiced at Supplier's current rate for propane in Consumer's type of equipment. In the event that Consumer does not exhaust Agreement gallons, any remaining credit balance on Consumer's account will be utilized to offset future propane purchases. No cash refunds of unused pre-purchase amounts will be made to consumer.

This Agreement shall be considered binding upon both Consumer and Supplier, their successors, assigns, designates, devisees, legatees, and heirs. This Agreement may be mutually terminated at any point in time upon written consent of both parties. This Agreement shall be considered null and void if not executed by Consumer and returned to Supplier within 15 days of Agreement Date.


Residential Rental Tank Agreement:
